Answer Jamie: Could you elaborate?  Are you saying that the driver opens even when the hardware is not attached?  What device is this?  
 
Well I could reproduce it once, but could not proove which driver. I had the following non standard drivers at that time, Roland U-8 USB, M-Audio Oxygen, Yamaha CBX (MU100) and maybe Yamaha UW500 (USB).
 
On a second approach the drivers had not been loaded, hence MIDI-OX did not crash.
 
On another machine I have the CBX driver but could not reproduce the problem with a switched off device.
 
I'm an on W2k SP3.
